Forms

You can view and download any form that you need to lease an apartment from Cardinal Properties. There is also information that you will need regarding care for your apartment.

Please download, print, read and fill out an Application, Lease and Addendum prior to your apartment showing. Bringing these forms completed, with every blank filled in, will expedite the process. If a blank does not apply to you, note why it does not apply.

If it is not appropriate or convenient to bring forms in person, they may be e-mailed or mailed to us.

New Applicant

1. Application -- This is the application that you will fill out if you would like to rent an apartment from Cardinal Properties.

2. Lease -- Every resident will sign a lease, which is an agreement between you and Cardinal Properties about what we expect from you as a resident and what you can expect from us as the property managers.

3. Lease Addendum -- Every resident also signs this lease addendum, which specifies that the resident will not have controlled substances on the premesis.

3. Lease Addendum for University of Minnesota students -- Every resident who is also a University of Minnesota student also signs this lease addendum, which gives an overview of local laws, University policy and possible penalties.

4. New Resident Checklist -- When you move in, you will receive a copy of this checklist to help you ensure that you have done everything that needs to be done as part of leasing an apartment.

5. Apartment Status Inpection Report -- This form should be filled and returned to Cardinal Properties as soon as you move in. If there are any issues with apartment, be sure to note them so that you do not incur any charges for them when you move out.

6. Utility Start Letter -- For your convenience, you may download and print a copy of the form to sent to the power company to start your service.

Resident

7. Extension of Lease Form -- As the end of your lease nears, you may fill out and submit this form to apply to extend your lease for another year.

8. Maintenance Request Form -- If something goes wrong in your apartment, please notify us with this form. We encourage you to use our automated online maintainance request, available by clicking on the Maintainance Request button on the lefthand menu.

9. Checkout Letter -- Prior to moving out of your apartment, you must clean thoroughly and leave it as clean as or cleaner than it was when you moved in. This letter detail steps that you must take.
